DI 11020.012 When an Application for Childhood Disability Benefits (CDB) is Not Needed

An application for CDB benefits is not needed if the child

  is currently entitled to child benefits on the account, see GN 00201.005B); or

• •

  was previously entitled to child benefits on the account, see Forms Used in CDB Claims DI 11020.025B.1.b.

For these cases, complete the SSA-3368-BK and other required forms per DI 11020.025B.
