GN 00205.040 Disability Question on Applications for Widow(er)'s Insurance Benefits (WIB)

Complete an SSA-10-BK or Modernized Claims System (MCS) equivalent and, an SSA-3368-BK (Disability Report--Adult) for a claimant for Disabled Widow(er)’s benefits (DWB). For more information about forms used in disability claims, see DI 11005.016.

Treat the disability question on the SSA-10-BK or MCS equivalent as a lead where the widow(er) is under age 61 and files for WIB but could establish entitlement to DWB during the retroactive period. For title II retroactivity, see GN 00204.030.
