GN 01704.820 Requests to Route National Forms

In some instances, the foreign agency will request that one or more of its national forms be sent to a person, completed and returned to the agency. For example, under the German agreement, the Seamen's Insurance Fund will frequently send an explanation and an application for direct deposit with their D/USA 3. If other development must be undertaken through a DO, the DO should also be asked to have the foreign agency forms completed. Enclose an
English translation of this form if it is available. If no DO contact is necessary, send the forms directly to the individual for completion. In either case, diary for 60 days. The completed forms should be returned to INTPSC for forwarding to the foreign agency.

If the forms are not attached to the foreign agency request, send the request back to that agency, and explain that the forms were not attached to their initial request. Do not diary for return of the request.
