GN 01726.205 Preparing Claims Packages in DIO for Claims Filed Outside the Netherlands

A. Introduction

Initial claims packages are prepared to request Dutch coverage and to inform Dutch agencies about claims for Dutch benefits. (See GN 01725.205-GN 01725.215 for the applications and evidence required for the packages).

B. Procedure

Follow the appropriate procedures below for preparing claims packages

1. Initial Claims Packages

Prepare initial claims packages to request Dutch coverage records and to inform the Dutch agencies about claims for Dutch benefits. (See GN 01725.205 - GN 01725.215 for the applications and the evidence required to prepare the packages.)

2. U.S. Claims

Prepare form SSA-2960-NL to request the Dutch coverage record, if one is needed. (See GN 01726.320 for disability claims).

3. U.S. and Dutch or Dutch Claim Only

Prepare a claims package that includes:

  • •

    Form SSA-296-NL

  • •

    U.S. totalization earnings record

  • •

    Medical evidence from DIO files, if the Dutch claim is for disability benefits

  • •

    Evidence submitted for the Dutch claim, and

  • •

    A request for the Dutch coverage record, if one is needed
