GN 01728.205 Preparing Claims Packages in OIO for Claims Filed Outside Portugal

A. Introduction

Initial claims packages are prepared to request Portuguese coverage certifications and to inform the Portuguese agency about claims for Portuguese benefits. (See GN 01727.205 - GN 01727.215, for the application and evidence required for the package.)

B. Procedure

1. Potential Entitlement to U.S. Totalization Benefits

If there is potential entitlement to U.S. totalization benefits, prepare an SSA-2960-U3-PE to request the Portuguese coverage certification.

2. Claim for Portuguese Benefits

If there is a claim for Portuguese benefits, prepare a claims package including:

  • •

    Form SSA-2960-U3-PE;

  • •

    U.S. totalization earnings record;

  • •

    medical evidence from OIO's files, if the Portuguese claim is for disability benefits; and

  • •

    evidence submitted for the Portuguese claim.
