A spouse, surviving spouse, or parent should file an SSA-760-F4 (Certificate of Support) within the specified time limit whether or not a claim is
filed.
An SSA-3 (Marriage Certification) must accompany the spouse's SSA-760-F4.
If the support determination is favorable, evidence supporting all allegations is
required.
For child's benefits, this form may only be used to determine whether the "other reasonable
period" rule or the "untoward circumstances" rule may apply when determining the period
of support for stepchildren (see RS 01301.020).
