Use the following forms when reentitlement is based on a disability which began after age 18:

D. SSA-3368 (Disability Report - Adult)

Always. (An exception applies if a concurrent Title II Disability Benefit (DIB) or Title XVI adult disability/blindness claim is being filed for a child. See DI 11005.037 and DI 11015.025.)

SSA 820/821 (Work Activity Report – Self Employed Person/ Work Activity Report – Employee)

If Work Activity Reported
