This table describes the action necessary for FO development of beneficiary inquiries
concerning the nonreceipt, replacement, or correction of cards issued by RRB.

Telephone the local RRB office so that an RRB card can be issued. |

Provide RRB with all pertinent data as follows:

If the inquiry involves loss or nonreceipt of card, provide beneficiary name, RRB
claim number, and address (indicate if this is a recent change).

If the inquiry involves correction of information on card, provide beneficiary name,
RRB claim number, and information to be corrected.

Inform the beneficiary that the new card will be mailed by RRB. |

Do not maintain a control on this contact. No response will be received from RRB. |
