2. Office of Information Services (OIS)

The Office of Information Services (OIS), Innovative Healthcare Delivery Systems Group (IHDSG) Division of Medicare Enrollment and Payment Systems is responsible for all Part B surcharge billing systems-related issues including data communication and data access. The Division of Medicare Enrollment and Payment Systems also prepares the monthly surcharge bills and investigates problem cases submitted by the surcharge agency. The address for this component is:

B. Policy-CMS Regional Office (CMS RO)

The CMS RO is responsible for acting as the initial contact for States and local governments interested in entering into a Part B premium surcharge agreement. The CMS RO will provide general information to States and local governments regarding the Part B Premium Surcharge Billing Program and can provide an information packet that contains the Part B Premium Surcharge Payment Handbook, the Formal Agreement and a Third Party Agency Information Form.
