If the claimant needs help securing evidence and another FO has more access to obtain it, request assistance from the servicing FO (FO you are requesting service from) to assist in obtaining the necessary evidence or information.

Example: The number holder (NH) needs five to six weeks to obtain evidence, but the servicing FO (field office that we are asking for assistance) can obtain the same evidence in one week. The NH’s local FO should request assistance from the servicing FO that has the capability to obtain the evidence quicker.

When requesting evidence from another FO, use form SSA-562-U3 Request for Assistance Certification or the Electronic e562 Request for Assistance/Certification of Documents or Records.

NOTE: When necessary, the SSA-562-U3 assistance request should include a check or money order for payment of any fees associated with obtaining evidence.
