a return envelope and payment. Telephone requests for all types of records, using
VISA or Mastercharge, can be made by telephoning (971) 673-1190. Fax requests can
be made to (971) 673-1203. The fee for a record request by phone or fax is $32.50.
Verification only is $8.00. Contact Oregon Center for Health Statistics directly using
the State of Oregon’s form CHS45–110. Oregon does not accept credit cards for verifications.
NOTE: Birth verifications are preferred evidence only if the birth occurred in 1939 or
later and the record is not marked delayed.
