The field office (FO) and Disability Determination Services (DDS) must complete entries in Item 11, as applicable, when making a finding of presumptive disability (PD) or presumptive blindness (PB) in a paper case. For instructions when the case is processed electronically see DI 23535.020.

Complete this section if either the FO or the DDS makes the PD or PB decision.

NOTE: Do not make an entry in Item 11A or 11B for a fully electronic case.

Complete Items 11A and 11B as follows:

  If the FO made the decision to process the case under the PD/PB procedures and did not complete item 11, check block 11A, enter code 01 in the lined space and enter in 11B, the applicable PD/PB category number from the chart in DI 11055.231.

  If the DDS makes the decision to process the case under the PD/PB procedures, check block 11A, enter code 02 in the lined space and make no entry in 11B.
