Form SSA-4514 is required quarterly from each agency.
Submit copies as follows:

the original will be sent to Disability Adjudication (DA)

the original will be sent to the SSA Central Office no later than the 25th day of
the month after the close of the quarter.

For example, for the quarter that ends 9/30/2026, reports will be due by 10/25/2026.
