Prepare a medical exhibits folder for each consultative examiner
as follows:
•
Use a letter-size
folder. On the front of the folder, write or type the claimant's
name and Social Security number, and the type of CE requested.
•
Make legible copies of the material and relevant
evidence identified by the ALJ as related to the type of examination
ordered along with the most recently completed disability report form:
i.e. the SSA-3441 (Disability Report-Appeals). Material that is
not relevant to the type of examination ordered should not be included.
