RM 00299.010 Form SSA-L670 Request for Evidence in Support of an SSN Application -- Foreign-Born Applicant

A. Use of Form

This letter is used to request evidence of age, identity, and U.S. citizenship or alien status from applicants for SSN's who allege to be foreign-born (i.e., any birthplace except those in the 50 States, District of Columbia, Puerto Rico, Guam, American Samoa, Swain's Island, and the U.S. Virgin Islands). (See details: RM 10210.010 through RM 10210.020)

B. Preparation of Form

    Enter DO Address.

  2. 2. 

    Enter applicant's mailing address.

  3. 3. 

    Check the appropriate block.

C. Other Actions

  1. 1. 

    If SS-5 has already been received, return it with the letter. Applicant should then resubmit the SS-5 and evidence together.

  2. 2. 

    If letter is being sent in response to a request for an SSN application, enclose a blank SS-5.
