SSA field offices, TSCs or other facilities may receive requests for volume supplies
of Form SSA-7004-SM-OP1 from employers, agencies, Congressional offices and other
organizations. If the request is for fewer than 500 copies of the form, the office
receiving the request should fill it from local stock.
(Offices should check their stock periodically to ensure that they have adequate supplies
of the most current version of the form. This is particularly important for FOs that
service Congressional offices.)
When the request is for 500 or more forms, take one of the following actions:

Offer the SSA-7004-SM (SPEC). When requesters are willing to reproduce the necessary supply of the form from a
facsimile at their own expense, send the request to:

The Design and Development Branch will mail a Form SSA-7004-SM (SPEC) to the requester
and will maintain a listing of all SPEC requesters so that they can mail new SPEC
forms to the requesters whenever we revise Form SSA-7004-SM-OP1.

When you forward these requests, include the following information:

the requesters name and telephone number,

•
the address to which the forms should be shipped,

•
an “ATTENTION”: line if they are to be sent to a specific person at that address,

•
a statement as to whether the requester wants the English or the Spanish version of
SSA-7004-SM-OP1,

•
the number of forms (in each language, if appropriate) that are required, and
-
•
any other pertinent instructions regarding shipment.
