Tell the individual we were unable to process his/her request because the name, date of
birth or sex provided on the request did not match our records.

Determine if the SSN on the holder’s SSN card is the same as the number shown on the SSA-7014A.

If no, go to step 4. If “unknown” (i.e., the individual does not have an SSN card), screen
for the correct SSN and go to step 7.

Ask the SSN holder if the name, date of birth and sex shown on the Form SSA-7014A are
correct.

Tell the requester:
-
•
to enter the correct information in the blank space(s) on the Form SSA-7014A,
-
•
to return the form to the WBDOC address shown on the form,
-
•
that, upon receipt, WBDOC will reprocess his/her request using the correct information,
and
-
•
an SSA-7005-SM-OR or SSA-7014A should be received within 3 to 6 weeks.

Obtain and develop an SS-5 and appropriate proofs to:
-
•
Update or correct our records to reflect the information provided by the requester, and
-
•
issue a duplicate SSN card.
NOTE: Even though the requester may not need a duplicate SSN card, receipt of the card
ensures that newly developed SS-5 information has been processed and is now reflected
on our records.

Give the requester a blank Form SSA-7004-SM-OP1 and tell him/her:
-
•
that we have updated our records to reflect the information the requester provided
-
•
that after the duplicate SSN card is received, to complete the SSA-7004-SM-OP1 using
the SSN, name, date of birth and sex used to update or correct our records,
-
•
to send the completed Form SSA-7004-SM-OP1 to the address on the envelope provided,
and
-
•
that we will process the form and send the requested Statement within 3 to 6 weeks.

If an SSN is located and the name, date of birth and sex match the name, birth and sex on the SSA-7014A:
-
•
obtain and develop an SS-5, and issue a duplicate SSN card,
-
•
use appropriate procedures to inform the SSN holder what the correct SSN is, and
-
If the SSN is located, but the name, date of birth and/or sex alleged on the request does not match our
records, go to Step 5.
If the SSN is not located and the SSN holder has not used an incorrect SSN in employment:
-
-
•
use appropriate procedures to inform the SSN holder what his/her correct SSN is, and
-
•
tell the requester since no SSN was ever issued or used in employment, there would
be no earnings credited to our records.
If the SSN is not located and the SSN holder has used an incorrect SSN in employment:
-
•
initiate SSN assignment, and,
-
•
use appropriate procedures to inform the SSN number holder what his/ her correct SSN
is, and
-
•
initiate an earnings discrepancy investigation,
-
•
give the SSN holder a blank Form SSA-7004-SM-OP1, and
-
•
tell the SSN holder to complete the form, and to send it to the address on the envelope
provided with it after he/she has been notified that the earnings discrepancy has
been resolved.
