Forms SSA-7014A that have been forwarded to the FO under cover of a Form SSA-984 with box B.4. checked involve SSNs that WBDOC determined through manual verification to be correct, but that will not pass electronic verification. (See RM 01301.515.)
Use the following steps to process these requests for assistance:

Contact the requester and explain that we have processed his /her request for a Social Security Statement, but the identifying information the requester provided in response to our request for additional information still does not match our records.

Tell the requester we need to update or correct our records to reflect the correct information so that future requests for statements will be processed by the system electronically.

Determine if the information shown on the most recent Numident record is correct.
If yes, go to Step 4.
If no, go to Step 5.

Determine if the requester wants to update or correct our records to reflect the name, date of birth and sex he/she provided in response to our request.
If yes, go to Step 5.
If no, go to Step 6.

Take the following action:
obtain and develop an SS-5 to update or correct our records,
give the individual the Social Security Statement processed by WBDOC, and
tell the requester that on future requests for a statement, to be sure to provide the name, date of birth and sex used to update or correct our records.
STOP.

Give the requester the Social Security Statement processed by WBDOC, and tell him/her that on future requests for a statement to be sure to provide the name, date of birth and sex shown on the most recent Numident record.
