Subchapter RM 01301.000 explains how SSN holders can request a Social Security Statement. Subchapter RM 01305.000 describes the automatic issuance of SSA Initiated (SI) Statements as required by section 1143(c) of the Social Security Act. This subchapter discusses
the form that SSA sends out in response either to a request or to selection for an
automatic Statement.
The Social Security Statement, Form SSA-7005-SM (in versions -SI or -OR), is SSA’s basic tool for providing SSN
holders with information about their earnings records and estimates of their potential
benefits. We redesigned the form — previously called the Personal Earnings and Benefit
Estimate Statement (PEBES) — to make it shorter, easier to read and simpler to understand.
We introduced the new design in October 1999 in conjunction with SSA’s expanded program
for annual issuance of statements to all workers age 25 and older.
NOTE: Under its old name, Form SSA-7005-SM was often referred to by its acronym “PEBES.”
The redesigned form has no approved acronym. You should call it the Social Security Statement or simply “the Statement” when discussing it with the public.
