RM 10205.135 NH Date and Place of Birth on Application for SSN Card

A. Date and place of birth required on all applications for an SSN card

The number holder’s (NH) date and place of birth are mandatory on every application for an SSN card.

If the date or place of birth provided by the applicant differs from what is shown on the Numident, we must see evidence to support the change.

B. Date or place of birth omitted or incomplete

When a Form SS-5 is received with an omitted or incomplete date or place of birth and the applicant is not in the office, take the appropriate action, as indicated for the following situations:

1. Application for an original SSN

If the application is for an original SSN and the date or place of birth is omitted or incomplete, use the date or place of birth shown on the evidence document used to establish age.

2. Application for a replacement SSN card

If the application is for a replacement card and the date or place of birth is omitted or incomplete, contact the applicant by telephone, if possible, for an allegation as to his or her date or place of birth. If there is a discrepancy between the allegation received and the Numident, or if the applicant cannot be reached by telephone, use an SSA-L669 (Request for Evidence in Support of an SSN Application — U.S.-Born Applicant) or SSA-L670 (Request for Evidence in Support of an SSN Application — Foreign-Born Applicant) to request missing documentation. Include Form SS-5 and any documents with the notice.
