RM 10212.085 Evidence Required to Process a Name Change on the SSN based on a Court Order Name Change

Evidence of the name change event, the new name, and identity is required to process a name change based on a court ordered name change.

A. Evidence of name change event

For acceptable evidence of a name change event based on a court order, see RM 10212.080.

B. Evidence of new name

The new name shown on the court order is evidence of the new name.

C. Evidence of number holder’s identity for a name change based on a US court order name change

Refer to RM 10212.015D and RM 10212.020 for evidence of identity requirements. There are no special circumstances related to court order for name change events.

D. Example of acceptable evidence to process a name change based on a US court order name change

Sam Spade requests a name change to John Pierce. He submits evidence of the name change event, his court order for name change, issued last week. The court order shows the new name, John Pierce and the old name, Sam Spade, and his birth date. In this case, the court order is also evidence of identity in the prior name, since the prior name shown agrees with the name shown on the latest Numident, it shows his date of birth and was issued within the past two years.

We can process the name change.
