ACCEPTABILITY OF HOSPITAL BIRTH RECORDS

The Texas Department of Health, Bureau of Vital Statistics (BVS) has given Texas hospitals authorization to use Form VS-125/175.7 (Certification of Birth Facts) when providing the public with extracts from hospital birth records.

In addition to the criteria in GN 00302.530B., the form must show direct evidence as to its origin by one of the following:

  • •

    The hospital certifier may reproduce the BVS form onto hospital letterhead and then provide an original signature as certifier; or

  • •

    The hospital or certifier seal may be affixed to the BVS form.

Accept properly certified Forms VS-125/175.7 as evidence of U.S. citizenship and evidence of age for claims purposes or alternate evidence of age for enumeration.
