Idaho requires a release signed by the subject of the record, or their representative
for a copy of a birth, marriage, or divorce record. A release signed by a family member
or the executor of the estate is required for a death record. Use of the SSA-L706
is preferred. Divorce and marriage records are available since July 1947. The subject
of the record or representative must include with the request a copy of photo ID with
signature or two non-photo IDs with signatures. The SSA employee shown in Part 4 of
the SSA-L706 does not need to submit evidence of identity. Requests for verification only of a vital record
do not require evidence of identity.
