Can FO verify, certify or obtain free copy of birth/death or marriage record? No
NOTE: If unable to verify a birth record through the Electronic Verification of Vital
Events, contact the Maryland BVS, which requires the individual's permission before
it will verify their birth record for SSA. Obtain a statement (e.g., on a Form SSA-795)
signed by the individual, and include the original signature (not a copy or fax) with
the request for verification. Also include either a copy of the BC to be verified,
or the file number of the BC, along with a copy of a present valid, unexpired, government-issued
photo identification (the identification must have a date issued and an expiration
date – see the Vital Statistics link above for more information).
Sample statement:
I give the Social Security Administration permission to verify my birth record with
the Bureau of Vital Statistics in order to complete the processing of my request for
___________________________.

The Maryland BVS also requires the Social Security Administration to verify the identity
of the applicant for the birth/death or marriage request. Include with the verification
request a statement that the applicant’s identity has been verified.
Sample Statement:
“I have verified the identity of the applicant”. Sign your name and title. Also print
your name and title. Indicate your field office name, address, and telephone number.
