GN 00605.100 When a Payee Evaluation Report (SSA-624-F5) Is Required

A. POLICY

The FO must conduct face-to-face interviews and complete an SSA-624-F5 when:

  • •

    A change in custody has taken place.

  • •

    The payee gave unacceptable responses on the accounting report which cannot be resolved (GN 00605.065 through GN 00605.068).

  • •

    No response is received after two requests for accounting (GN 00605.085).

EXCEPTION: In limited nonresponse situations, telephone contact is sufficient to document a Report of Contact (SSA-5002). (See GN 00605.085B., Step 3, for specific situations).
