GN 01710.210 Preparing Claims Packages in DIO for Claims Filed in Switzerland

A. Introduction

Initial claims packages are prepared to request Swiss coverage records and to send information to the Swiss agency.

B. Procedure

Follow these steps to prepare a claims package.

1. U.S. Claim

  • •

    Prepare a claims package if a Swiss coverage record is required and has not been requested by the FSP in Switzerland.

  • •

    Include Form SSA-2960-U3-SZ to SSA-e2960-U3-SZ in the package.

2. U.S. and Swiss or Swiss claim only

Prepare a claims package if:

  • •

    the Swiss coverage record is required and has not been requested by the FSP; or

  • •

    the Swiss agency has requested information or documentation.

Include the following in the claims package:

  • •

    Form SSA-2960-U3-SZ to SSA-e2960-U3-SZ; and

  • •

    information or documentation requested by the Swiss agency.
