GN 01710.316 Controlling Requests for Special Swiss Applications- Procedure

To control requests for special Swiss applications, do the following:

  • •

    Diary all requests for the special Swiss applications mentioned in GN 01710.315 for 60 days.

  • •

    If there is no response to the initial request, send a second notice and diary for an additional 60 days.

  • •

    If the form is received send it to the Swiss agency via Form SSA-e2960-U3-SZ and check the second block in Part V of the form.

  • •

    If there is no response to the second request, inform the Swiss agency via Form SSA-e2960-U3 SZ that the claimant has not responded to two requests for completion of the Swiss application form.
