GN 01716.230 Controlling Requests Sent to Canadian Agencies on Form SSA-2960-U3-CQ

Procedure

Prepare a diary when information is requested using the following:

  • •

    Diary all initial requests for 90 days

  • •

    Diary the second request for 30 days.

  • •

    Diary the third request for 15 days.

Provide the dates of the previous requests on all follow-ups.

Fax the SSA-2960 CA to (613) 952-8901
            SSA-2960 QC to (418) 643-4809

Only mark the request as "Urgent" if the claimant is terminal, in dire need or no response has been received after the 3rd request. Send these requests to the attention of Celine Leduc in Ottawa fax # (613) 952-8901.
