RS 01403.014 Signed Statements From Employers — Policy

Signed statements by the employer or an authorized person of the employer that are based on the employer's records or known facts are acceptable as primary evidence if such statements have the same essential information as appears on Forms SSA-7011-F4, SSA-1002-F3, and SSA-1003-F3.

The statements need not be addressed to SSA but should be on company letterhead, if possible, and/or should indicate authority of the individual signing the statement.
