The interviewer or reviewer is responsible for reviewing the forms for completeness, accuracy and clarity.
NOTE: No questions should be left blank. If the claimant does not remember or does not know the answers, indicate on the form that the information is unknown.
When reviewing:
Ensure all items and questions have been completed to the extent possible.
Enter “don't know” or “not applicable” where needed to indicate that the question was not inadvertently omitted.
Make any changes directly on the form when it is being completed during a face-to-face interview.
Record modifications or supplemental information on a Report of Contact when the claimant has already completed the form. Indicate which item or question number(s) is being modified or supplemented.
