DI 26510.020 Completing Item 17 (Diary) on the SSA-831

A. Introduction

Individuals receiving disability benefits must continue to meet the disability requirements of the law. To ensure this, SSA conducts continuing disability reviews (CDRs). Under the law, all disability beneficiaries must be reviewed at least once every three years, unless they are permanently disabled. Permanently disabled beneficiaries must be reviewed on a schedule determined to be appropriate by the Secretary. The frequency of scheduled review is determined by the likelihood of medical improvement of the impairment(s).

  • Medical Improvement Not Expected (MINE) cases involve permanent impairments and are reviewed at least every seven years.

  • Medical Improvement Expected (MIE) cases and Medical Improvement Possible (MIP) cases involve nonpermanent impairments and are reviewed after a shorter period.

The diary date for periodic medical review is indicated in Item 17 of the SSA-831.

B. How to complete the diary entry (Item 17) on the SSA-831

NOTE: The Disability Determination and Transmittal, SSA-831 requires month and year entries for diary dates. However, the NDDSS requires that a day entry of "01" also be entered for diary dates. The diary date can contain four or six characters depending on the Legacy system used by a particular State DDS; e.g., 08/05; 08/01/05; 08/2005.

Complete Item 17 of the Disability Determination and Transmittal, SSA-831 on all allowance cases as follows:

C. Entering the diary for Medical Improvement Expected (MIE)

1. Diary type

Enter "MRN."

2. Diary date

  • Establish an appropriate diary date 12 to 35 months in the future.

  • Enter a four-digit code for month, and year (MM/YY), counting the month of adjudication (e.g., if the month of adjudication is 08/07, and the diary is set for 24 months, set the diary at 07/09).

3. Diary reason

Enter an appropriate MIE diary reason code. For diary reason entries, see “MIE Criteria, DI 26525.025B.

D. Entering the diary for Medical Improvement Possible (MIP)

1. Diary type

Enter "MRN."

2. Diary date

Enter a four-digit code for month and year for 36 months (3 years) in the future; count the month of adjudication (e.g., if the month of adjudication is 3/05, set the diary at 02/08).

3. Diary reason

Enter "3."

E. Entering the diary for Medical Improvement Not Expected (MINE)

1. Diary type

Enter "MRP."

2. Diary date

  • Enter a four-digit code for month and year for 84 months (7 years) in the future; count the month of adjudication (e.g., if adjudication occurs in 11/99, diary for 10/06).

  • In all claims, establish the diary date without regard to the claimant's actual age (even if the diary will not mature until after the person attains full retirement age).

F. Closed period and favorable determinations for deceased claimants

  • Enter "NXP" for the diary type.

  • Enter "0000" for the date.

  • Enter "7" for the diary reason.

G. Trial Work Period (TWP)

1. General

Enter the appropriate medical or vocational diary data. See Item 34 for the TWP entry.

Enter the month after the month the vocational therapy is expected to end.

Enter a four-digit code for the month and year (MM/YY).
