DI 28095.030 Spanish Continuing Disability Review (CDR) Language Notices
If the disability determination service (DDS) determines that you need a Spanish notice,
the remark “Spanish Notice Required” will appear on the forms:

SSA-831-U3 (Disability Determination and Transmittal),
-
•
SSA-832 (Cessation or Continuance of Disability or Blindness Determination and Transmittal
– Title XVI), or
-
•
SSA-833 (Cessation or Continuance of Disability or Blindness Determination and Transmittal
– Title II).
Send an English notice with a Spanish cover sheet. Prepare the personalized attachment
in English in all cases (except in Puerto Rico). The DDS prepares the notice according
to the instructions in NL 00603.110, Spanish Language Notices and NL 00610.080 Spanish Notices. For special notice situations involving Spanish language notices

Steps to follow for Spanish notices:

Prepare the English notice following existing instructions.

2.
Enter the name, address, claim number, and date (same date as English notice and entered
after the word “FECHA”) on the Spanish language cover notice.

3.
Attach the Spanish notice to the front of the English notice. Include both the English
and Spanish versions of the appropriate pamphlets. When applicable, mail the letters
to the beneficiary or recipient or their representative.

4.
Attach a copy of the Spanish cover notice to the copy of the English notice and file
both in the claim folder.
