Field components may receive complaints about the TPM. If this occurs:
Identify the individual making the complaint (e.g., a beneficiary, EN, State VR agency, other service provider, State P&A specialist);
Take a signed statement from the individual on Form SSA-795; and
Forward the Form SSA-795 and any accompanying documentation the individual wishes to submit, to the Regional Center for Disability Programs for subsequent forwarding to ORDES.
NOTE: If the individual submits a letter or other statement complaining about the TPM, forward it to the Regional Center for Disability Programs.
