FOs should request birth and death record verifications by mail or fax directly from
the Vital Records Branch.
Complete and sign the Form SSA-L706 (see GN
00302.070). This form must accompany the employee identification letter (see GN 00308.001B.3) and a copy of the ID of the parent or the person who's applying. Include a legible
copy of the birth certificate, if available. The SSA-L706 form is to be signed by
the SSA employee and by the person(s) whose information is being requested. Requests
for verification should be sent to the following address: DC Vital Records 2201 Shannon
Place SE 1st Floor Washington, DC 20020. Main number is 202-442-9289.
There is a $5.00 fee for verification and a $10.00 fee for an uncertified copy.

FOs should request birth, death, marriage, and divorce records by mail directly from
the Division of Vital Records. The request must indicate the number of the check or
money order and the FO's return address. Use SSA letterhead and include the following
