Requests to change or correct name information on SSA records sometimes accompany requests for earnings statements.

The responsibility for changing or correcting name information on SSA records is not under the jurisdiction of this procedure. As a courtesy to the requester, forward Form SS-5, Application for Social Security Card, to the requester to enable the number holder to change or correct his/her records.

If an earnings request is received with a request to change or correct name information:

  Annotate “SS-5 Sent” on top of the original Form SSA-7004-SM-OP1 and sort for data entry.

  Highlight the necessary instructions on Form SS-5 and forward to the requester.

  Include the Correspondence Transmittal, with the SS-5. Check box 4 “Other”, on the in-house form and annotate “We have processed your earnings request and should receive the information in approximately four to six weeks.”

If an earnings request is not received with a request to change or correct name information, mail Form SS-5 to the requester. Highlight the necessary instructions on Form SS-5 to change or correct name information. Forward the original correspondence received requesting the name change back to the number holder with the SS-5.
