Follow the steps below for forwarding copies of completed Forms 3857.

Complete a Form SSA-L54, Form Letter to IRS, (original only) to transmit completed
Forms 3857.

Address the form to the IRS office that submitted the Form 3857, using the address
provided in the upper left corner of the form.

Attach each completed Form SSA-L54 to the related Form 3857 and release for mailing
daily.
