GN 01722.106 How to Evaluate Spanish Coverage Certifications

A. Introduction

There are some entries which must be shown on the form E/USA 4 before it can be used to credit Spanish coverage. This section describes the entries required for an acceptable Spanish coverage certification.

B. Procedure

Check the coverage certification form for the following entries:

  • •

    Worker's name

  • •

    "FROM-TO" entry for each period in which there is an entry for days.

  • •

    “DAYS” entry for each “FROM-TO” period and “DAYS” entry that is less than or equal to the number of days in the corresponding “FROM-TO” period.

NOTE: When checking the number of days in the “FROM-TO” period, use 30 days for each month or partial month shown in the “FROM-TO” period.

  • •

    Spanish agency stamp, signature and date.

If any of the entries is not shown, route the case to IOTS for evaluation.
