GN 01722.210 How OIO Prepares Claims Packages for Claims Filed In Spain

A. Introduction

Claims packages are prepared to request Spanish coverage records and to respond to Spanish agency requests for information.

B. Procedure

Use these instructions for preparing claims packages.

1. U.S. Claim

Prepare a claims package if a Spanish coverage record is needed and has not been requested by the FSP. Include the following:

  • •

    Liaison form SSA-2960-U3-SP.

  • •

    Form E/USA 2 (Statement of Working Life).

2. U.S. and Spanish or Spanish Claim Only

Prepare a claims package if:

  • •

    A Spanish coverage record is needed and has not been requested by the FSP; OR

  • •

    A Spanish agency has requested information or documentation.

Include the following in the claims package:

  • •

    Liaison form SSA-2960-U3-SP

  • •

    Information or documentation requested by the Spanish agency.
