GN 01722.310 Processing Spanish Agency Requests for Medical Evidence and Examinations

A. Process

If a Spanish agency requests medical evidence, OIO will forward medical evidence from OIO files. OIO will not request medical evidence from other PSCs. The Spanish agency request is submitted on form E/USA 3.

If a Spanish agency requests a medical examination from a U.S. resident, SSA will schedule the examination and send the results to the Spanish agency. The Spanish agency request is submitted on form E/USA 5.

When a Spanish agency requests medical evidence, send the evidence from OIO files as described in GN 01704.810C.

Follow these steps when responding to a Spanish agency request for a medical examination.

Step	Action
1	Request a foreign disability examiner to schedule an examination, or forward “pertinent medical evidence from files” as described in GN 01704.810
2	Stamp the first and last page of each medical record being sent to Spain with “true and exact copy.” Sign and date the blocks on the stamp.
3	Request reimbursement for the cost of medical examinations obtained for the Spanish agency using form SSA-1278-U3-SP. Refer to the instructions in GN 01704.810B. for completing the form.
