Follow the procedures below to prepare an initial claims package, to request an Irish
coverage record and to send information to the Irish agency. Prepare a claims package
if:

an Irish coverage record is required and has not been requested by the FSP in Dublin,
and

•
include Form SSA-2960-U3-IR in the package.
2. U.S. and Irish or Irish claim only

an Irish coverage record is required and has not been requested by the FSP; or

•
the Irish agency has requested information or documentation, and
-
•
include Form SSA-2960-U3-IR, and
-
•
information or documentation requested by the Irish agency.
