The Japanese agencies will send detailed disability forms, called Certificate of Diagnosis, directly to claimants for Japanese disability benefits. While those forms are to be completed by the claimant’s doctor and forwarded directly to the Japanese agency requesting the form, claimants may return the forms to OIO by mistake.

 

If this occurs, return the form to the requesting Japanese agency using form SSA-2960-J.
