When an applicant files concurrent claims, only one disability report needs to be
            completed. In the situation in which adult concurrent claims are filed, the SSA-3368
            should be completed. In the situation in which a title XVI disability or blindness
            claim is filed for a child (i.e., an individual who is under age 18) concurrently
            with a title II disability claim (which would normally require completion of adult
            forms), the FO is responsible for obtaining an SSA-3820 and an SSA-3379.
