Use either Form SSA-1407 or some alternative routing form (e.g., a route slip) to refer for VR. If the Form SSA-1407 is used, only items 1, 2, 2a and 3 of that form need to be completed.

Attach legible copies of the following material:

1. a. 

   Form SSA-1994, Cover Sheet (Confidential Information).

2. b. 

   The Disability Determination, Form SSA-831-U5, or Continuance or Cessation of Disability, Form SSA-833-U5.

3. c. 

   Forms SSA-3368-BK, SSA-3369-BK and SSA-3820-BK.

4. d. 

   Current medical reports pertinent to the diagnosis.

5. e. 

   Vocational evaluation reports.

6. f. 

   Any other information considered useful for rehabilitation purposes.

7. g. 

   Any material relating to a plan for achieving self-support.
