This section discusses field office (FO) and teleservice center (TSC) procedures for
            assisting the public and WBDOC in handling requests for Social Security Statements, Form SSA-7005-SM-OR, that contain:
         
         
            - 
               
                  •
                  
                     no SSN or an incomplete, impossible or illegible SSN,
                     
                   
                
             
            - 
               
                  •
                  
                     an SSN or identifying information that does not match SSA records, or
                     
                   
                
             
            - 
               
                  •
                  
                     other factors that result in issuance of a Form SSA-7014 to the requester.
                     
                   
                
             
         
         The problems may surface while WBDOC is reviewing and keying request Forms SSA-7004-SM-OP1,
            or the Statement 2000 system may identify them and send a SSA-7014 letter to the requester,
            asking for corrected information.
