RM 10205.090 Form SS-5 Received and Additional Documentation is Needed
When you cannot process a Form SS-5 (Application for a Social Security Card) because
the application is incomplete, the proper evidence is not included with the application,
or a mandatory in-person interview is required, send the applicant written notice
explaining why the SS-5 was not processed and what we need to process the application
for an SSN card.
When returning Form SS-5 and any documents to the applicant, use the appropriate form
letter as follows:

If the applicant was born in the U.S., use an SSA-L669 (Request for Evidence in Support of an SSN Application — U.S.-Born
Applicant)

If the applicant was born outside the U.S., use an SSA-L670 (Request for Evidence in Support of an SSN Application — Foreign-Born
Applicant).
The applicant must resubmit Form SS-5 along with all required documents at the same
time.

Indicate on the SSA-L670 that the applicant should bring DHS documents to an FO, SSN
Card Center, or contact station rather than putting the documents in the mail.

Age 12 or older, requesting an original SSN card
|
Indicate that the applicant must visit an FO, SSN Card Center, or contact station
for an in-person interview.

|
Applying on behalf of another
|
Indicate that the applicant must also submit evidence of their own identity and evidence
that shows their relationship and authority to apply for the SSN on behalf of the
NH.
