RM 01301.015 SSA-7004-PC-SM Form Numbers, DOC Addresses and Geographical Areas of Responsibility

A. Procedure — distribution of individual forms SSA-7004-SM-OP1 to the public

People who want to request a Social Security Statement can get a paper SSA-7004-SM-OP1 request form by calling the SSA toll-free 800 number, by calling, writing or visiting their local SSA office, or by downloading the form from SSA’s Internet website. The office receiving the request will mail Form(s) SSA-7004-SM-OP1, in English or Spanish as appropriate, to the requester.

NOTE: Requests may also be transmitted electronically using the online version of the SSA-7004-SM-OP1 on SSA’s Internet web site (www.socialsecurity.gov ).

B. Procedure — requests for larger supplies of request forms

SSA field offices, TSCs or other facilities may receive requests for volume supplies of Form SSA-7004-SM-OP1 from employers, agencies, Congressional offices and other organizations. If the request is for fewer than 500 copies of the form, the office receiving the request should fill it from local stock.

(Offices should check their stock periodically to ensure that they have adequate supplies of the most current version of the form. This is particularly important for FOs that service Congressional offices.)

When the request is for 500 or more forms, take one of the following actions:

  • Offer the SSA-7004-SM (SPEC). When requesters are willing to reproduce the necessary supply of the form from a facsimile at their own expense, send the request to:

The Design and Development Branch will mail a Form SSA-7004-SM (SPEC) to the requester and will maintain a listing of all SPEC requesters so that they can mail new SPEC forms to the requesters whenever we revise Form SSA-7004-SM-OP1.

  • If the requester will not accept the SPEC, send the request to the supply warehouse in Baltimore at the following address:

When you forward these requests, include the following information:

  • the requesters name and telephone number,

  • the address to which the forms should be shipped,

  • an “ATTENTION”: line if they are to be sent to a specific person at that address,

  • a statement as to whether the requester wants the English or the Spanish version of SSA-7004-SM-OP1,

  • the number of forms (in each language, if appropriate) that are required, and

  • any other pertinent instructions regarding shipment.
