RM 01340.030 Examining the Social Security Number (SSN) Holder's SSN

A. Introduction to SSN holder’s SSN

Use the procedures in this section when examining the number holder's SSN provided on correspondence or on Form SSA-7004-SM-OP1 (Request for Social Security Statement).

B. Policy for processing an SSN request

Before we process an SSN request, the requester must provide the number holder's SSN anywhere on the correspondence or on Form SSA-7004-SM-OP1.

C. Description of illegible, impossible, or incorrect SSNs

1. Illegible SSN

An illegible SSN is one that you cannot readily determine from the incoming request.

2. Impossible SSN

An impossible SSN is one that meets one or more of the following criteria:

  • The SSN begins with a “9” or the first three digits (former area number) are “000” or “666”.

  • The second group of digits (former group number) are “00”.

  • The third group of digits (former serial number) are “0000”.

  • There are more or less than 9 digits.

3. Incorrect SSN

An incorrect SSN is one that is manually or electronically determined to be incorrect, or that does not match the name indicated on Form SSA-7004-SM-OP1.

D. Procedure for examining the Social Security Number (SSN) holder's SSN

Examine the correspondence or Form SSA-7004-SM-OP1, and take the following actions:

  1. Determine if the requestor provided the number holder's complete SSN in item 2 on Form SSA-7004-SM-OP1, or anywhere on the request.

  2. If a complete SSN shows anywhere on Form SSA-7004-SM-OP1, follow procedures on examining the SSN holder’s date of birth in RM 01340.031.

  3. If a complete SSN does not show anywhere on the form, and if a telephone number

    • is available, obtain an alphadent and perform direct contact, following procedures in RM 01340.042.

    • is not available, return the request under cover of Form SSA-L220, (Social Security Requests for Earnings Information). Check box 1.

  4. Place a red checkmark next to the SSN box on the original Form SSA-7004-SM-OP1.

  5. Enclose the following in an envelope and mail to the requester:

    • the original Form SSA-7004-SM-OP1 (Request for Social Security Statement), and

    • Form SSA-L220 (Social Security Requests for Earnings Information).
