RM 01340.031 Examining the SSN Holder's Date of Birth
The requester must provide the number holder's date of birth.
A request is not acceptable for processing if the number holder's date of birth is not shown anywhere on the request. The date of birth consists of the month, day, and year of birth.
However, consider a complete date of birth if only the month and year are provided. If the day of birth is blank, annotate the day of birth as “02”.
C. When to use
Use this procedure when examining the number holder's date of birth provided on the Form SSA-7004-SM-OP1 or correspondence.
Examine the request and make a determination if the number holder's date of birth is provided. Take the following action:
If you determine a complete date of birth is provided, continue processing per RM 01340.032.
If the date of birth is not provided, and a telephone number is shown, view the numident record and contact the requester using direct contact procedures in RM 01340.042 to obtain the SSN holder's missing DOB.
NOTE: Do not provide the contact with any of the information shown on the numident.
If a telephone number is not provided, return the request under cover of Form SSA-L220. Check box 3.
Place a red checkmark next to the DOB box on the original SSA-7004-SM-OP1.
Determine if the number holder is within four months of attaining age 62 or older, and expresses an intent to file a claim for retirement benefits.
If an intent to file a claim is not shown, continue processing per RM 01340.032.
