RM 01340.039 Examining the Request for a Spanish PEBES

A. Introduction

A PEBES in the Spanish language may be requested by checking the appropriate box on Form SSA-7004-SM (English) or by completing a Spanish-language Form SSA-7004-SM-SP (Spanish).

B. Policy

Unless the requester indicates he/she wants the statement to be in Spanish, the statement will be in English. Therefore, this is not a required entry on the request.

C. When to use

Use this procedure when the requester submits a Spanish Form SSA-7004-SM-OP1, checks the box on the English Form SSA-7004-SM-OP1 or indicates that he/she wants the PEBES in Spanish.

D. Procedure

If the request is for a Spanish PEBES, take the following actions:

  • If a Spanish response is indicated, code “SL” in the special indicator box on Form SSA-7004-SM-OP1 and continue processing per RM 01340.039.

  • If a Spanish language Form SSA-7004-SM-OP1 is received and the number holder checked the box, do not treat as an “SL” special. When the box is checked on a Spanish Form SSA-7004-SM-OP1, the number holder is requesting that the earnings information be in the English language.

  • If a Spanish language Form SSA-7004-SM-OP1 is received and the number holder did not check the box, treat as an “SL” special. The number holder is requesting that the earnings information be in the Spanish language.

  • When correspondence is received in a foreign language and requires translation, complete part A of Form SSA-533, Translation Request. Annotate your clerk and unit number on the bottom portion of the Form SSA-533.

  • Annotate the following address on the top of Form SSA-533 under “TO”.

  • Annotate the following address on the bottom of Form SSA-533 under “FROM”.

  • SSA Wilkes-Barre Operations Center
    1150 East Mountain Dr., Room 431
    Wilkes-Barre, PA 18702

  • Photocopy all material and staple the copies together. Give both the original and copies to the Administrative Clerk in your assigned units. The Administrative Clerk will be responsible for mailing all translations to the above address.

  • The Administrative Clerk will diary each case for 35 work days. If no reply is received after 35 days, contact the Program analyst at extension 1591.
