RM 01340.040 Examining the Signature

A. Introduction

The requester must sign the request. The signature is usually provided in the blocked area at the end of the Form SSA-7004-SM-OP1 or correspondence.

B. Policy

A request is not acceptable for processing if not signed. In addition, the signature must appear after the statement certifying that the requester is the individual to whom the record pertains and that he/she understands the criminal penalties for knowingly and willingly requesting information from Social Security records under false pretenses. Assume the signature is that of the requester unless you have reason to suspect otherwise.

C. When to use

Use this procedure when examining the requester's signature as entered on the Form SSA-7004-SM-OP1 or correspondence.

D. Procedure

Examine the signature on the Form SSA-7004-SM-OP1 or correspondence and handle as indicated.
